NCPDP Updates

Specialty Medication Prescribing PSS update - Specialty Medications
- June 22, 2020
  - OVERVIEW of IG
    - focuses on getting information related to a patient/prescription prior to dispensing a specialty medication
    - Looked at daVinci CDex Implementation Guide
    - Solicited request
      - pharmacy receives a prescription (electronically)
      - upon receipt - need more information to fulfill the prescription
      - create request bundle using Communication/CommunicationRequest
        query for one or more of Condition, MedicationStatement, AllergyIntolerance, Observation (vital signs), Coverage
        this should be MedicationRequest and not MedicationStatement
      - CDex is looking at changing from Communication/CommunicationRequest to Task -
        decision will be made this summer
    - Responder will send back a bundle with information requested
    - Pharmacy may have more in depth questions - may leverage Questionnaire for follow on information
    - Patient matching - Looking for information on best practices
      - CDex doesn't talk about patient matching
      - Looking at 2 options
    - There are some constraints on US Core
    - Plan to ballot in September cycle - need to get the NIB submitted
      - also means needs to be substantively complete by July 5th
      - Frank to reach out to Melva about getting the NIB submitted

  - White paper may be abandoned - NCPDP to make a decision
  - may move forward with creating a FHIR IG - no update

US Core

review change to errata request FHIR#25035
- FHIR-25035 - when recording "self-prescribed" medications SHALL include requester - concern that you wouldn't always know this for self-prescribed medications
  - proposed changing from SHALL to SHOULD
    - if don't know, then wouldn't provide it
    - this only applies to "self-prescribed" medications and not actual orders
  - Need the blessing of the Pharmacy
  - Motion: Melva Peters - Brett Marquard - Pharmacy endorses this change 9-0-0 Carried
  - Will now go to Cross Group Projects

Real Time Pharmacy Benefit Check
- June 22, 2020
  - Publication Request is complete - Consumer-facing Real time Pharmacy Benefit Check Publication Request
    - IG is complete - with no errors and warnings
    - all reconcilations have been applied
    - Motion by Frank - seconded by Pooja to accept Publication Request - Carried 9-0-0
    - Melva will forward Publication Request to the TSC for approval
- June 15, 2020 - update via Email from Frank
  - US Core usage
    - The Cross Group Projects WG discussed our US Core exception tickets and approved them with the following adjustment:
      - dosageInstruction in the MedicationRequest resource (the ‘prescription’ in the RTPBC request) will be changed to Must Support, with the following definitions:
        Client: Should use the information when determining days supply
        Responder: May use the information when determining its response
      - I’ve made the changes to the IG and will be moving to the build site soon
  - NDC11
    - We are now referencing the NDC in the guide using the correct URL and our profiles, examples are okay
    - (The guidance will probably get better in the future, but HL7 doesn’t have it yet)
  - Other terminology stuff that FM is working on…
    - Got permission from FM to move forward as-is (without changing to the approach that’s being defined)
