- Specialty Medication Prescribing PSS update - Specialty Medications

- OVERVIEW of IG
- focuses on getting information related to a patient/prescription prior to dispensing a specialty medication
- Looked at daVinci CDex Implementation Guide
- Solicited request
- pharmacy receives a prescription (electronically)
- upon receipt - need more information to fulfill the prescription
- create request bundle using Communication/CommunicationRequest
- query for one or more of Condition, MedicationStatement, AllergyIntolerance, Observation (vital signs), Coverage
- this should be MedicationRequest and not MedicationStatement
- query for one or more of Condition, MedicationStatement, AllergyIntolerance, Observation (vital signs), Coverage
- CDex is looking at changing from Communication/CommunicationRequest to Task -
- decision will be made this summer
- Responder will send back a bundle with information requested
- Pharmacy may have more in depth questions - may leverage Questionnaire for follow on information
- Patient matching - Looking for information on best practices
- CDex doesn't talk about patient matching
- Looking at 2 options
- There are some constraints on US Core
- Plan to ballot in September cycle - need to get the NIB submitted

- this only applies to "self-prescribed" medications and not actual orders
- Need the blessing of the Pharmacy

- The Cross Group Projects WG discussed our US Core exception tickets and approved them with the following adjustment:
- dosageInstruction in the MedicationRequest resource (the ‘prescription’ in the RTPBC request) will be changed to Must Support, with the following definitions:
- Client: Should use the information when determining days supply
- Responder: May use the information when determining its response
- I’ve made the changes to the IG and will be moving to the build site soon
