Description of Ballot Document:This implementation guide specifies an electronic initial case report (eICR) to support public health reporting of reportable conditions from healthcare organizations to public health agencies are required by law. 
Major Changes Since Last Ballot:STU comments will be addressed; update guidance and narrative descriptions, add blank question/answer capabilities for unknown questions in future outbreaks, add space for reportable condition from Reportability Response, add solution for EHR data entered in error, update some cardinalities, add disability element, add other requested elements

The following information is required by ANSI for all ballots.
Project NeedCase reporting is part of the CMS and ONC NPRMs for Meaningful Use Stage III. Electronic case reporting is considered to be a critical public health need. An electronic initial case report (to PH) and a returned reportability response (to healthcare) are critical components of a viable nationwide approach. The failure to have a workable case reporting standard will impede outbreak management and negatively impact the monitoring of disease trends.
