Short Description

Identify where to find and how to query "real-world" test data needed for birth reporting via the Birth and Fetal Death Reporting (BFDR) and Vital Records Common Profile Library (VRCPL) FHIR implementation guides (IGs).

Long Description

The scope of BFDR IG includes the content of medical/health information on live births and fetal deaths for select state birth and fetal death reporting. It includes the content that is exchanged between EHRs and jurisdictions that issue birth certificates, as well as between jurisdictions and national vital statistics agencies.

The 2023-01 track will focus on querying and retrieving data for test patients loaded into EHR/EMR test environments (sandboxes) needed to complete the Facility Worksheet for the Live Birth Certificate for reporting from the birth facility to the jurisdiction. (Scenario 1: EHR-to-EBRS workflow)

BFDR Track goals:

  1. Identify where/how data needed for birth reporting (specifically the Provider Live Birth Report) are held and coded in various EHR/EMR systems. Ideally, each participating system will have an EHR/EMR sandbox to query. This will help find additional codes used by various EHR/EMR systems for each data element.  
  2. Provide feedback on the narrative/explanatory content (what's missing, what's confusing) of the published IG (http://hl7.org/fhir/us/bfdr/) for hospitals/EHR/vendors and jurisdictional vital records office electronic birth registration system (EBRS) implementers.

This track will support efforts underway by the Vital Statistics Modernization Community of Practice (CoP) of the CDC's National Vital Statistics System (NVSS).

Multiple perspectives wanted:

  • Knowledge of how EHR/EMR systems code birth-related data and where those data are stored
  • Vendor sandbox test environments that can load test birth records (see 3 Test Cases below) or already contain similar live birth test case data
  • Jurisdictions that can receive Facility Worksheet for the Live Birth Certificate data via the FHIR Composition - Provider Live Birth Report

System Roles

  1. EBRS/"birth clerk" that queries the EHR/EMR for data required to complete the Birth Certificate for submission to state jurisdiction
  2. EHR/EMR data source server
  3. Jurisdictional Vital Records Office’s Electronic birth registration system (EBRS) data receiver
  4. Optional system role = SMART-on-FHIR App: This optional actor may extract data from a data source and format and transmit the FHIR bundle

Testing Scenario

This track defines 2 scenarios:

  1. EHR-to-EBRS workflow with manual query
  2. EHR-to-EBRS workflow with SMART-on-FHIR App query

Scenario 1: EBRS vendor (acting as "birth clerk") queries EHR/EMR system to assemble data for the Facility Worksheet for the Live Birth Certificate for reporting from the birth facility to the jurisdiction by creating and sending the Composition - Provider Supplied Facility Live Birth Report

Scenario 2: SMART-on-FHIR App queries EHR/EMR system to assemble data for the Facility Worksheet for the Live Birth Certificate for reporting from the birth facility to the jurisdiction by creating and sending the Composition - Provider Supplied Facility Live Birth Report

Action:

  1. EBRS (or interface engine for "birth clerk") or SMART-on-FHIR App:
    1. Queries the EHR for the source mother and baby's birth information. 
    2. Uses the source information to create the Composition - Provider Facility Live Birth Report consisting of required data (SHALL data elements) on the newborn patient, related participants and birth certificate data
    3. Sends the Composition - Provider Facility Live Birth Report to the EBRS
  2. The EBRS verifies the Document Bundle against the BFDR IG using the FHIR Validator

The sub-page, BFDR: Birth Reporting Test Case Data provides 3 test cases may be loaded into testing environments (sandboxes) for EHR, EMR, or HIE systems. If the testing environments already have data for similar live birth events, loading these data is not required for the January 2023 Connectathon.
