Interoperability, SDoH, Opportunities for Program Improvement
Several states are leading the way in implementing person-centered care models and piloting interventions for Medicaid enrollees using SDoH data, and coordinating services through their MCO (Managed Care Organization) partners and CBOs (Community Based Organizations). At the same time, states are implementing the information-blocking rule and payer-to-payer data exchange models in order to be compliant with CMS rule-making. The convergence of these efforts, and the maturation of services being provided through HIEs, which are emerging as critical public health utility partners, promise to provide a trusted HHS data exchange framework that will not only improve the coordination of beneficiary services but address the needs of communities, as a long term strategic imperative.

Typically, two or three organizations will showcase their work and answer audience questions, allowing for a vibrant discussion and fostering industry understanding of the Da Vinci work products and resulting market solutions. Improving Patient Outcomes Using FHIR: How providers are exchanging clinical data with payers to deliver better quality of care
May’s Community Roundtable focuses on bidirectional clinical data exchange between payers and providers using Da Vinci’s PDex and CDex implementation guides. This month’s implementation story from FloridaBlue, healow insights and Humana discusses how standardized information sharing improves patient care. The session includes a demonstration of clinical data exchange capabilities, such as point of care alerts and up-to-date patient histories, underscoring the value of using FHIR standards to optimize electronic medical record integration to assist in achieving value-based care outcomes. Rounding out this month’s webinar will be Da Vinci program updates and use case milestone progress including highlights from HL7 Connectathon and WGM+.

How Provider-Payer Collaboration Maximizes Value Based Performance Reporting (ID #810)
The HL7® Da Vinci Project is a multi-stakeholder collaborative effort engaging providers, payers and partners with the goal of accelerating the adoption of HL7 FHIR® (Fast Healthcare Interoperability Resources) as the standard to support and integrate value-based care (VBC) data exchange in real time. As fee for service shifts to value-based care, value-based contracts have emerged as a mechanism that providers and
payers may use to better align their contracting structures with broader changes in the healthcare system.
This session provides you with front-row access to HL7 Da Vinci Project’s newest use case that aims to develop an HL7 FHIR Implementation Guide to support value-based performance reporting for quality and risk contracts. Proposed by a provider, this use case seeks to solve challenges around the lack of reporting format standardizations, the resource-intensive process, lack of scalability and the complexities of data
reconciliation.
After a brief high-level perspective regarding the value-based performance reporting component, presenters will highlight the genesis, development, project scope and progress of the use case. The opportunities to tackle the challenges of gathering performance data will be a focus as well as how you can leverage the HL7 Da Vinci project Implementation Guides to improve your reporting.
