The new 2021 IPPS NPRM is out and available for comment. There don't seem to be any changes to public health reporting, but it may be useful for organizations to comment that there should be changes including stronger requirements for public health reporting. Please review and work within your organizations to comment as appropriate

Methodology

eCR Now Implementation Update (Laura Conn)

  • PHWG eCR Status update 5 29 2020.pptx
  • COVID19 is reportable in all jurisdictions
  • eCR Now has had a dramatic effect:
    • Over 400,000 case reports have been triggered from over 2000 facilities in 17 healthcare organizations in 30 jurisdictions
    • Reporting at this sites is now faster and more complete
  • There is still a good queue of organizations to do the rapid onboarding

  • eCR Now HL7 Connectathon Report Out - PHWG v2.pdf
  • The May virtual connectathon included the eCR Now FHIR app
  • The app handles some of the EHR challenges with th eCDA eCR
    • outputs eICR 1.1
    • ingests trigger updates via eRSD
    • handles the reportability response (RR)
  • Initial software releases on May 1st and 8th
  • The connectathon included working sessions for testing and error/defect logging
  • 5 successful app implementations
  • Cerner presented their work in a production ready EHR instance (after having had the app after about 3 days)
  • Next steps
    • production release by May 31st including DSTU and R4 support
    • Direct and XDR transport
    • RR sorting clinically relevant documents to the patient chart
    • Trigger code management
  • The team is getting EHR and health system commitments to implement
  • John will be speaking to EHRA next week
  • Utah expects the eCR will save significant time for Public Health both in terms of FTE work and the speed of reporting
MethodologyCancer reporting IG update (Wendy Blumenthal)
  • The project team is close to finishing the updates to the Cancer reporting IG based on the previous 2 sets of errata
    • They are ready to publish from Trifolia along with the supporting documentation
    • We will work to publish the updated document as a dot release
    • A dot release doesn't restart the STU clock (it will still expire in March 2021)
  • Other non-errata updates are pending and will be worked on once the errata version is published
    • The original PSS is still active
    • These are more significant changes and likely warrant a re-ballot
      • Diagnosis section will get some modelling changes to better represent the data content
      • A few new data elements
      • Possibly some value set updates
      • Some header updates
    • This will come back to the WG for a formal decision on a dot release versus reballoting
  • The current IG is based on C-CDA on R2.0
    • If the next release goes to ballot, should it be upgraded to R2.1?
    • Danny indicated that as general rule of thumb, going with R2.1 would be preferred
    • There may be impacts on regulatory requirements
    • The question could be posted to Zulip on the CDA stream for additional input
MethodologyRevisit VRDR US Core decision (Rick Geimer, Sarah Gaunt)
  • Sarah spoke with Brett about not using US Core profiles as the basis of an IG
    • The WG doesn't have to approve it, but the project team needs to provide the rationale to either US Realm or the CGP WG
