DICOM Structured Report (DICOM SR) is a standard for recording clinical imaging observations made regarding an diagnostic or interventional imaging procedure. Imaging Observations are made by humans, such as a sonographer making measurements on recently acquired ultrasound image, a Radiologist recording observations on an suspected legion, or by a machine, such as an automated AI Algorithm providing qualitative and quantitative observations.
DICOM SR is widely adopted by Imaging-based devices and IT systems. Non-imaging based Healthcare IT Systems, generally, do not support DICOM SR. Non-imaging healthcare systems support HL7 standards, such as FHIR. Bridging the two standards for clinical imaging observations becomes necessary for interoperability between these type of systems.
The outcome of this project will be one or more implementation guides with defined sets of mappings between SR content and FHIR resources.
The mapping will not be bi-directional -- a complete DICOM SR is not expected to be able to be re-constructed from a set of FHIR resources.

The outcome of this project will be an implementation guide with a defined set of mappings that will convey the imaging observations as a FHIR observation. This IG will be bounded by the following expectations:
Imaging observations, captured as DICOM SR objects will be mapped to FHIR Observations.
Observations, captured as FHIR observations are not expected to mapped to DICOM SR Observations.
ImagingStudy resource will be used to reference the study.
ImagingSelection will be used to reference the image or region of interest on the image.
DICOM Part 16 defines a number of structured report templates – identified by a Template ID (TID) – corresponding to clinical use cases. The primary DICOM SR template for this IG is TID 1500. TID 1500 is the SR templated profiled by IHE for AI Results (AIR) TI Supplement. Mapping of AI Results to HL7 v 2 observations without the granular reference to the observation on the image is profiled by IHE as part of the AI Workflow for Imaging TI Supplement. This IG will compliment these IHE profile supplements with FHIR observations. These supplements provide for the basic use case of this IG.
