Social Risk Data Submissions


The Gravity Project identifies coded data elements and associated value sets to represent social determinants of health (SDOH) data documented across the following four clinical activities: Screening using Assessment Instruments, Diagnoses, Goals, and Interventions (Procedures and Service Requests). The work focuses on identifying, developing, and validating 1) the data elements needed to document SDOH data across all four clinical activities, and 2) national standards to support the electronic capture and exchange SDOH data across a variety of systems and settings of care and social services. The following formal submissions have been made to coding stewards in order to address the social risk data concept gaps identified during the domain work.  Codes that are accepted by the respective standards developing organizations will be published in upcoming releases.

Gravity Published Social Risk Data Elements


The dashboard contains hyperlinks to domain-specific value sets for Assessment Instruments, Diagnoses, Goals, and Interventions (Procedures and Service Requests). For Assessment Instruments, VSAC has a limitation in reflecting screening question and answer pairs and their association with a panel (Assessment Instrument) code. To address this limitation, the spreadsheets in the last column below provide links between panel, question, and answer codes. For an explanation of the fields in the Assessment Instruments spreadsheets, please see Assessment Instruments Spreadsheet Guidance document here (link pending).

Gravity Project social risk data elements are published in Value Set Authority Center (VSAC) value sets. The value sets can be identified by searching for "The Gravity Project" steward. You will need to create a free National Library of Medicine (NLM) account to access the value sets. After completion of initial updates to this page on April 31st, 2023, value sets will be updated bi-annually on June 30th and December 31st.

    Hi All - would be great if this nice dashboard view was updated to include some of your (recent?) additions (e.g., Elder Abuse Procedures using CPT and SNOMED etc..).

      Caroline, thank you for your comment. VSAC (the value set authority center) is the home of all up-to-date references for diagnoses, goals, and interventions/procedures. The VSAC value sets are updated regularly. Of note, we recognize an opportunity to clarify this in our introduction paragraph! I hope this helps. Sarah
