2021-01 Care Coordination Track | This track will be hosted by the Multiple Chronic Conditions (MCC) eCare Plan Project and the Logica Care Management Project. The goal is to continue testing of the MCC eCare plan FHIR IG and SMART on FHIR Apps (provider and patient-facing) and the Logica Health Care Management project with guidance on model-driven design and execution of clinical guidelines and pathways.
