3. Project Definition

3.a. Project Scope


Scope: This project will review and analyze the substance and extent of clinician burden in the US and international healthcare communities, particularly burdens associated with use of electronic health record (EHR) systems, other health information technology (HIT) systems, and the exchange/use of electronic health information. The ultimate objective is to reduce clinician burden (RCB). The foremost focus for the HL7 EHR Work Group is to reduce clinician burden associated with EHR system functionality and how that functionality is described/specified in standards such as ISO/HL7 10781 EHR System Functional Model (EHR-S FM) Release 3 - the next major iteration of the EHR-S FM. Broader focus is for HL7 WGs in development of standard specifications for health information (including data definition, data structures (data types and formats), vocabularies, code and value sets, exchange, security, privacy, provenance, integrity, traceability, content, context, relevance, timeliness, actionability) and how such specifications will aid in reducing clinician burden, now and in the future.

The Project Team will review many reference sources, including trade publications, professional society journals, articles, studies and personal experience, to document the characteristics and magnitude of clinician burden. The Project will then identify specific recommendations and RCB success stories to guide formulation of EHR-S functions, functional statements and conformance criteria for EHR-S FM R3. The Project will also identify specific recommendations that may guide other HL7 WGs on how to address RCB in developing their specifications, such as standards related to health information capture, exchange and use.

The Project Team will reach out to the international community (beyond the US realm) for input regarding the challenge of clinician burden.

The challenge of clinician burden (and burnout) is widely documented in the healthcare community. The need (on the part of HL7 and other standards developers) is to understand the substance and extent of clinician burden and devise effective approaches to address the burden in the formulation and promulgation of EHR/HIT standards.
