Q: If we use our own vocabulary, do we have to host our own structured data capture also?R: Don't generally need to host structured definitions, the rule is if you are defining the extentions, then the rule is they need to be publicly available. Options would be to Host or put in a Registry. 

  • other note: May want value sets, but they don't need to be exposed unless you want others to use them.


Q: Formulary - Once a patient logs in, how do we connect them to their formulary?

R:  Options: 

  • Through PDex coverage resource, then an internal mapping to get them to the appropriate coverage and coverage plan.
  • Behind the scenes internal mapping (outside FHIR) - then you wont have the dependency between coverage plan and coverage resource. 
  • can also be one prescription drug plan offered to members, no choice, so just return the single drug plan.

Two separate questions: Can we get this data?  Can we publish that data? 

  • Trying to encourage providers to add digital end points into the NPI record.  As we see more adoption, we will see more end points information available to share.  
  • Now, you just need to prove you share it, if you have it. 
  • CMS - need to share if you have it. 
  • CAQH national FHIR end point directory - ONC FAST team focused on Payers, but will likely extend into the future for provider end points as well.  


Question on #017

  • Can publish additional values from within the guide. e.g. drug tier.
    • has own plan-specific value set. 
    • how do we add a drug tier so the consumer understands it and can validate it. 
  • There is a drug tier value set, but it's extensible. 
  • Deductible - could be zero or could have a maintenance drug that is covered before the deductible, but others go through the deductible. 
  • Value that is missing is a charge value - drug not subject to deductible, but there is a deductible otherwise. (not free, but not subject to deductible)
    • "Charge" isn't clear - so something like "excluded from deductible" or "outside of.."
    • "not subject to deductible" - people liked this. 
  • Let's harmonize with the EOB. 
  • This is in the drug tier definition extension. 
  • Two uses: co-pay and co-insurance. 

David Hill- create a ticket and propose an update on this for next week.  


Cannot use the url for code system.  Just has to be a name space or identifier but doesn't have to be a resolvable. 

If you are sharing Drug Tier Foo from my ZYX code system, you display it to the end user. 

Requirement is to map codes to a standard code, you must send the code from the binding.  you can also send local codes, but must send standard codes.
