Ballot and Connectathon Schedule


Administrative Updates
  • Working through Infrastructure Steering Division feedback on Project Scope Statement 
    • Clarifying scope re: IHE alerting profiles - alerting from devices to personnel - excluded from our scope
  • CommunicationRequest, Messaging, Task - Lloyd McKenzie is still gathering feedback on his data sharing guidelines proposal
    • Assuming that we're using FHIR Messaging for Notifications IG
    • Lloyd's document likely doesn't change that approach


Connectathon 
  • Philadelphia Connectathon Hosted by Independence Blue Cross - December 11-12
  • Reference Implementation exists for this track
  • Jason Teeple/Cigna is planning to get a team together to participate in the Notifications track in Philadelphia
  • Gini McGlothin - BCBSAL will not be participating in the Notifications track in Philadelphia
  • Need folks to test sending to intermediaries, sending to multiple sites, security, etc. - not just testing content of message
  • CMS planning Connectathon in January
  • IHE Connectathon is a must for anyone participating in HIMSS Interoperability Showcase
    • Is participation at IHE a must for organizations participating in Da Vinci booth outside of Interoperability Showcase?
    • Da Vinci is not in the Interop Showcase at HIMSS20. We are in an independent booth, so we’re not bound by the IHE Connectathon requirement.



IG Review
  • Changed name from Alerts to Unsolicited Notifications
  • Removed references to Communication
  • Changed guidance in narrative part for this first pass of revisions
  • Conceptually, want to provide equivalent of ADT content in the message so recipients can do what they do with it now in terms of transition of care - where the patient is and why
    • We want everything that would be available in a discharge summary - not sure if this would be V1
    • PMO discussions with NCQA re: transitions of care measure and others
  • Is this a one-time communication, or several exchanges of communication to retrieve all the information? (i.e., WIll the communication include it all up front, or just send some of it and refer to other bits that receipient has to come back and retrieve)
    • If we wanted to send all, we'd send Encounter - trying to find a balance that is usable by many and provide what's needed
    • Talking about happy path use case for right now - sending a small set of resources,  80/20 rule
    • Normally when you exchange information, you send every resource that's being referred to - if Encounter refers to Patient and Practitioner, would expect Patient and Practitioner resources to be there, and not have to go ask for it
    • All the information is captured in the Encounter as references
    • Not trying to reproduce ADT V2 messaging
    • What does send 'everything' mean? Everything in message header, etc.
    • Proposed "rule": any resources referenced in the bundle, need to be included in the bundle - this is the model people are used to now
  • These are pushed/unsolicited notifications
    • Need to be easy to filter
    • Filter on admission type, discharge type, etc.
    • Todd Johnson chat comment: Recipient has no influence on the payload
  • Ulrike Merrick , Eric Haas , Linda Michaelsen and team will discuss further and bring additional ideas back to group next week
