Hundreds of people from patient groups, health systems, government agencies, vendors, payers, and others have participated. The CoP gathers to discuss, share and learn about recent achievements regarding CodeX FHIR standards and applications in the real world being developed through CodeX and elsewhere.

Advancing Genomics Data Exchange Using HL7 FHIR at the Molit Institut

In the realm of genomic testing, most data presently is sent from the laboratory to health care organizations in the form of a PDF, which isolates this non-computable information within the EHR and limits access to the data for clinical care and research.

Patrick Werner of the Molit Institut, presented on their use of FHIR to free the data. As a small independent non-profit research organization in Germany, Molit has received FHIR genomic reports for more than two years. Werner shared the opportunities, challenges and lessons learned with Molit’s successful genomics data exchange, including strategies implemented to ease data standards adoption across participating labs.  Werner shared Molit’s virtual tumor board for molecular oncology to showcase how Molit structures data and identifies possible gaps to assist identification of variants, prognosis on variants and recommendations.  


The GenomeX Opportunity: Exploring the Art of the Possible

Having a standardized means to gather genetic intelligence in a machine-readable format will provide countless possibilities.  During this presentation, MITRE’s May Terry shared her insights into how interoperable genomic data sharing can improve the quality and safety of patient care by enhancing research efforts and enabling precision medicine at the point of care.  She  provided a brief update regarding the latest GenomeX progress and highlight several opportunities for genomic data sharing within CodeX use cases, including prior authorization, integrated trial matching and clinical quality measures.

Quality Measures for Cancer: Public Call

The team demonstrated that impactful insight can be made if mCODE and other FHIR Implementation Guides are leveraged by an ecosystem of health organizations.

Use Case Background:

The CodeX Quality Measures for Cancer use case asks the question, “can oncology quality measures be effectively authored and executed using mCODE and FHIR, given that current measures must be manually collected and abstracted?” The project has assembled a team of leading healthcare professionals from ASCO, ASTRO, Telligen, Evernorth, and MITRE to create a solution that demonstrates the ability to author and evaluate digital quality measures using FHIR standards, along with mCODE profiles and extensions, for value-based programs and clinical quality improvement in the oncology domain.

The use case kicked-off less than one year ago, and the team has already authored and tested two proof of concept ASCO antiemetic electronic clinical quality measures (eCQMs) using mCODE, FHIR, and the Quality Measures Implementation Guide (QM IG).

The use case is initially acting as an exemplar showing what can be achieved by leveraging quality-focused FHIR Implementation Guides to create and roll out new, innovative quality measures.

Collectively Building the Next Generation of Quality Measures: A Discussion with the ONC on Quality Standards and Leveraging USCDI+ Quality

Kyle Cobb and Lisa Wagner of the Office of the National Coordinator for Health Information Technology (ONC) will join the July CodeX Community of Practice to discuss the development of the United States Core Data for Interoperability Plus (USCDI+) Quality initiative.  They will share use cases and updates on collaborative efforts with Centers for Medicare and Medicaid Services (CMS) and the Health Resources and Services Administration (HRSA), among others, to support the USCDI+ Quality initiative.  They will then engage in a discussion with the CodeX community regarding USCDI+ Quality data elements and any potential gaps as they plan for the next generation of quality measurement.  

CodeX Quality Measures for Cancer: Use Case Overview and Next Steps

To complement the discussion, Anthony DiDonato, senior healthcare analyst at MITRE, and Gail Winters, integrations architect at Telligen, will share an overview and early successes from the Quality Measures for Cancer use case, which aims to highlight new quality measures that could be developed – and new insights that could be made – if the minimal Common Oncology Data Elements (mCODE) FHIR Implementation Guide (an open standard language for cancer data) and other FHIR Implementation Guides were implemented and leveraged by an ecosystem of willing health organizations. The ultimate goal is to provide a path to meaningful data collection with reduced burden.

Dr. Amyé Tevaarwerk, senior associate consultant, Medical Oncology, and associate professor, Mayo Clinic Cancer Center, presented Capturing minimal Common Oncology Data Elements (mCODE) in Routine Clinical Practice, providing insight into success realized at the Mayo Clinic after implementing and adopting mCODE to improve capture and reporting of routine clinical care in medical oncology. Their work scaled beyond the initial pilot group, resulting in robust data available near real-time to inform patient care today and into the future. Dr. Tevaarwerk discussed pilot successes and challenges and how this work may be applicable to improving routine care at your health site. 

Ellen Anderson, enterprise architect, and Anup Mahansaria, healthcare interoperability leader, at eviCore provided an update of Prior Authorization in Oncology Use Case. Data standards, including mCODE and the HL7 Da Vinci Project FHIR Accelerator Implementation Guides, support the use case proof of concept with synthetic patient data. The lessons learned and pilot plans discussed will likely resonate with all who experience prior authorization challenges, especially those preparing to meet prior authorization federal mandates.

Project Updates:

The RTTD team recently completed Phase 2 of work – which consisted of participating at an Integrating the Healthcare Enterprise-Radiation Oncology (IHE-RO) Exchange of Radiotherapy Summaries (XRTS) Workshop in May 2021.  

 

The focus of the XRTS Workshop was to test and validate the radiation therapy end of treatment and on-treatment summary information that has been defined in the CodeX RTTD project and leveraged in the XRTS technical specification. This testing was successfully achieved during the XRTS Workshop. Varian and RaySearch implemented the mCODE and CodeX radiotherapy profiles in a test version of their systems and successfully sent the radiation therapy summary information associated with five different synthetic patients to Epic and RaySearch, within a test version of their electronic health records. 

 

Purpose:

The RTTD team is hosting a Public Call for the broader health community to (1) share recent project achievements and (2) ask if other organizations are interested in and would like to collaborate on future phases of work, as we continue pressing forward in 2022.

Each month we are holding a Public Call to advise of the latest endeavors happening in this use case to move the needle on automating, via industry data standards including FHIR, the prior authorization process.  The use case Champions are on focused on these efforts in order to reduce burden for all stakeholders but more specifically to improve care for cancer patients.

June 23-25, 2022Practical Big Data Workshop

The Practical Big Data Workshop series brought together global innovators in radiation therapy and diagnostic imaging who are leading in development and use of big data and artificial intelligence to improve care for cancer patients.

This year’s workshop was organized around guidance for clinical implementation of Big Data and AI driven approaches for: 

  • Genomics and Biomarkers
  • Image categorization and segmentation
  • Dosimetry including treatment planning and quality assessment

The workshop combined lectures, lively point-counter point debates, round table discussions, high top - laptop presentations, and expansive socializing to build both community and science.

Quebec SmartCare Consortium - Steering Committee Meeting

The Quebec SmartCare Consortium is bringing together key Quebec players in patient-centered data, mobile health technologies (mHealth), and AI research in order to demonstrate the value of SmartCare.

The Consortium is being led by the Opal Health Informatics Group at the Research Institute of the McGill University Health Centre. The consortium is made up of a group of public and private partners in Quebec that are working to build data-sharing technology centered on Opal.
