Documentation of an issue showing what can happen when value sets include the wrong members. This shows there are codes within SNOMED CT that represent the concept "Allergy to X" where the notion of "allergy to" is recorded in the semantics of the code. However, in C-CDA the information model holds the notion of "allergy to" and the template only requires the notion X to be supplied. Thus, many coded concepts within SNOMED CT must be filtered out or not selected for use in the value set bound to the model element where X is represented.
This issues has also appeared recently in working being done within the Gravity Project while addressing what coded concepts should be used with the ServiceRequest.code element. It also was decided that codes which include the concept "Referral to" shall not be included when defining the value set to be bound to this data element. Both "referral to y" and "referral for x" (where y= the program and x = the service) will not be used.
